Day Program Forms
Here are the forms you will need to help plan your day trip.
- Complete Day Education Programs Manual (PDF) - This document contains all the forms, program descriptions and curriculum connections.
Or you can print off only those documents you need; they've been broken up into separate documents below.
- Program Selection Form (PDF) – A day at Scanlon Creek Nature Centre consists of two ½ day programs or one all day program. Please fill out this form and fax it to Dan Williams at least two weeks prior to your visit.

- Health Form (PDF) – Health information for each student needs to be available at the Nature Centre. Existing forms may be brought, or our health form may be filled out by each student. NOTE: These forms remain with the visiting teacher and are returned to the school at the end of the day.
- Equipment List (PDF)– Programs proceed rain or shine. Please make sure each student's parent/guardian gets a copy of this list so they can be prepared for the weather.
- Map to Scanlon (PDF) – Please give a copy to your bus driver to facilitate a timely arrival. Advise the bus driver that upon entering the park they should follow the signs to the Nature Centre, not the Education Centre.
